Tournament Forms for ALL Teams
Each player on the team MUST complete these forms and acquire a parent's signature to be eligible to play at the tournament. Print this PDF Form, fill it out and then mail it (to the address below) or submit it when you register on the day the tournament starts.
If your team is using guest players, there is a maximum of 6 guest players allowed per team. Each guest player on the team MUST complete this form listing the details of each guest player for the guest player to be eligible to play in the tournament. Print this PDF Form, fill it out and then mail it (to the address below) or submit it when you register on the day the tournament starts.
If your team is traveling to Wisconsin from out of state, please begin the process of acquiring permission to travel from your State Association. In some cases, it can take more than 30 days to acquire. Links for states surrounding Wisconsin are below.
